Download an IADD Membership Application

To view and print the membership application in a new browser window, click on the membership application graphic below.

To view the membership application as a PDF document, click the "Get It" button (requires Acrobat Reader).

To download the membership application as a PDF document, RIGHT CLICK the "Get It" button, then select "Save Target As..." (Internet Explorer) or "Save Link As ..." (Firefox). (This will allow you to download and save the file on your computer for viewing and printing - requires Acrobat Reader.)
